Registration Procedure
The Following forms need to be submitted on your initial visit:
1. Student Registration Form
2. Emergency Card
3. Copy of Immunizations
After 30 Days of enrollment we will also need:
Health Care Summary Form
Additional Forms for Infants (6 weeks – 15 months):
One of the Following:
Dietary Instructions 1-3 months old
Dietary Instructions 4-7 months old
Dietary Instructions 8-11 months old
Dietary Instructions 12-16 months
1. We can fax a request to your provider on your behalf
2. Print one here and fill it out Child’s Immunization
3. Simply bring in a copy of immunizations
