All children will need the following forms on their initial visit:

I. Immunizations Form (options below)

1. We can fax a request to your provider on your behalf

2. Print one here and fill it out Child’s Immunization

3. Simply bring in a copy of immunizations

II. Student Registration Form

Print one here and fill it out at home!

III. Emergency Card (This form needs to be filled out at the center)

IV. Health Care Summary Form  *Not needed until 30 days after your initial visit.
